The study aims to identify environmental variables that influence the management efficiency of public hospital institutions in Paraná. Data Envelopment Analysis and regression analysis were used for data treatment and analysis. The studies indicated that the general average efficiency was 70% and that only 23.39% of the sample reached efficiency. Facing the environmental variables, results showed that the Municipal Human Development Index (HDI) and population density have no association with hospital efficiency. On the other hand, the proportion of elderly in the population, financial transfers from the Unified Health System (SUS), Gross Domestic Product (GDP), basic sanitation, and the region where it is located influence efficiency.
