determinants associated with the prevalence of exclusive breastfeeding in infants assisted by three health services in Niterói, RJ

Introduction: The decline in exclusive breastfeeding rates in the last decade highlights the need for attention to the topic and reinforcement of its importance by health professionals and the literature. Even with the recognition of the benefits of breastfeeding by several studies and bibliographies, there is still much to investigate about factors that influence the maintenance of the practice. Objective: To identify the determinants that may interfere with the maintenance of exclusive breastfeeding. Methods: This is a cross-sectional observational study using questionnaires in three units of the Unified Health System in the municipality of Niterói. Results: An association was found between exclusive breastfeeding and on-demand feeding, non-use of bottles, initiation of prenatal care in the first trimester, absence of problems during breastfeeding, municipality of residence, higher level of education, absence of complications during childbirth, use of pacifiers, and the room used for sleeping. Some variables did not show an association, such as: maternal age, primigravida, smoking mothers, use of postpartum contraceptives, paternal encouragement during breastfeeding, employment status, information about exclusive breastfeeding during prenatal care, difficult access to health services, breast milk in the first 24 hours, family income, public or private prenatal care, rooming-in, and marital status. Conclusion: We face an adherence to exclusive breastfeeding that is still far from ideal, given that the issue involves great complexity. The importance of investing in education and access to quality health services is evident, as well as the formation of a broad support network, both family and healthcare-related.
